What an emergency guardianship
proxy is:
This document appoints a temporary guardian for your
children should you become incapacitated, unable to
care for them yourself, or upon your death until permanent
guardianship is appointed.

- No one has legal authority for the well-being
of your children in the event you are temporarily
unable to care for them
- The wrong person has legal authority for your
children in the event you are temporarily unable
to care for them
- Confusion and strife over who should care for
your children if you are temporarily unable to care
for them
How an emergency guardianship
proxy protects you:
It allows you to choose the person who will care
for your children in case of an emergency, instead
of allowing other family members and friends to make
this decision for you if an emergency happens. Without
going to court, your Emergency Guardian will be able
to make medical decisions, keep your children enrolled
in school, and help your child with any financial
issues that may arise.
